Leadership Task Force Assessment
Please carefully read each of the eighteen descriptive
statements about various aspects of Leadership Task Force. Rate
the statement in terms of how frequently LTF engaged in the
practice described, by drawing a circle around the number that
corresponds to the frequency selected. Additional comments you
wish to make should be provided at the end of the survey. You are
given five choices:
1. If LTF RARELY or NEVER does what is described in the
statement, circle the number ONE.
2. If LTF does what is described ONCE IN A WHILE, circle the
number TWO.
3. If LTF SOMETIMES does what is described, circle the number
THREE.
4. If LTF does what is described FAIRLY OFTEN, circle the number
FOUR.
5. If LTF does what is described VERY FREOUENTLY or ALWAYS,
circle number FIVE.
In selecting the response, be realistic about the extent to
which LTF actually engaged in each activity. Do not answer in
terms of how you would like to see LTF engage in the activities
or in terms of what LTF should be doing. Answer in terms of how
LTF typically engaged in each activity.
Please respond to every statement. The usefulness of this
information depends upon your honesty in assessing the frequency
with which LTF actually engages in meeting its objectives and
activities. Remember, this is not a test and all responses are
strictly confidential.
